PRIMARY FUNCTION:

The Housing Case Manager provides support and education to clients with a disability such as mental illness or chemical dependency during their transition into housing at Mino-Bimaadiziwin Apartments to increase long-term stability in the community and avoid future periods of homelessness or institutionalization. There are 24 units designated to Supportive Housing; and services may be provided in the client's home or in the community. Services provided will strongly emphasize personal goal setting and developing strategies to achieve them, skill building, client advocacy, and connecting clients with community resources. Reports to Clinical Supervisor. Full-time position with benefits, Salary; DOQ

ESSENTIAL RESPONSIBILITIES & DUTIES:

Direct Client Services 50%

• Provide community-based services to eligible adult clients (age 18+) with a disability that impacts their ability to live successfully in the community.

• Meet with individuals 2-5 hours a week in the community, shelter or in the individual's home.

• Ensure that services are connected to the person-centered housing plan that is developed with each client and delivered in a manner that is culturally sensitive appropriate following a holistic and person-centered delivery method.

• Identify and assist with resolving barriers to accessing and maintaining housing.

• Support the person in applying for all benefits (county, state, tribal and federal) that will help them afford their housing.

• Educate client and stakeholders on how benefits, i.e., Housing Support (GRH), MSA Housing Assistance help someone pay their rent.

• Help the person to understand housing rules and expectations, and understand and negotiate a lease.

• Support with walk-through checklist; such as the living arrangement is safe and ready for move-in.

• Identify resources to cover moving expenses and to access household goods.

• Support the person to maintain housing through prevention and early identification of behaviors that may jeopardize their continued housing.

• Offer resources and referrals for additional mental health supports such as Mental Health Target Case Management and Mental Health Therapy, for identified challenges and behaviors impacting the client.

• Assist with the housing recertification process both for rental assistance and in negotiating the continuation of a lease.

• Train and coach on being a good tenant, good neighbor, lease compliance and household management.

• Support the person to understand and maintain income and benefits necessary to retain housing. Assist the client to set up automatic payments for rent and utilities.

• Provide program and client transportation as needed.

• Participate and assist in the tasks associated with client care including evaluation of client safety and appropriate interventions related to their functioning and safety.

• Act as an advocate for clients as needed.

• Support client with accessing crisis assistance through Clinical Supervisor or Director

Administrative 50%

• Participate in supervision, under the direction of designated supervisor.

• Meeting productivity guidelines, and proactively seeking referrals as needed to maintain a full case load.

• Maintain billing and documentation in accordance with Department of Human Services guidelines, and submits billing according to procedures established by department: daily progress/encounter notes, billing encounters, monthly progress reports, and file reviews in health records (either paper or electronic).

• Input data into the Homeless Manager Information System (HMIS) and Procentive in a timely manner for reporting and funding purposes.

• Provide client level data to Supervisor to be included in monthly program reports.

• Attend trainings and conferences to comply with all continuing education requirements, and further enhance technical and programmatic knowledge, such as:

o Complete mandated reporter training, which includes training on vulnerable adult law.

o Complete mandated Housing Transition and Housing Sustaining training.

o Adult Mental Health Crisis and de-escalation training, Module 1-6

• Abide by and complies with the rules of confidentiality, Data Privacy Act, mandatory reporting laws, HIPPA requirements, and organizational policies and procedures.

• Attend community meetings as requested by the Supervisor.

• Support with related administrative tasks such as completing purchase requests, ordering supplies, and paying bills.

• Perform other duties as assigned.

EDUCATION/WORK EXPERIENCE/QUALIFICATIONS:

• At least 21 years old.

• High school graduate or equivalent.

• Current Minnesota driver's license, clean driving record, auto insurance, and regular and consistent access to a motor vehicle.

• Willingness to complete extensive training during onboarding and receive ongoing training in mental health, housing/homelessness, public health and public welfare, and Federal, State, and Tribal programs in these fields in order to provide advocacy and enhance patient's functioning and access to identified community resources

• Knowledge and experiencing working with American Indian/Alaskan Native clients preferred

• Proven ability to engage compassionately with clients

• Ability to maintain calm and professional response in stressful situations

• Utilize harm-reduction and person-centered approaches with clients

• Strong time management and task prioritization skills

• Ability to work independently with minimal direction and within part of a team.

• Ability to work creatively and collaboratively with colleagues internally and externally

• Proficient user of electronic health record systems for documentation and communication
