PRIMARY FUNCTION:

Play a support role to the psychiatrist in medication management of mental health conditions. Reports to Director of Mino Bimaadiziwin Wellness Clinic. Part-time position. Salary $40/hour; DOQ and experience.

ESSENTIAL RESPONSIBILITIES & DUTIES:

• Consult with providers to assist with ongoing management of chemical health and behavioral health needs

• Partner with the client, their family and significant others to develop treatment plans that address identified goals

• Triage clients by assessing severity of symptoms and referral to appropriate setting or level of care

• Alleviate suffering through nursing interventions and prescribed treatments

• Educate clients on their conditions and necessary medications

• Educate client and family in preventative care which promotes health and well being

• Document interventions and an individualized plan of care

• Coordinate services with the internal and external medical and behavioral providers

• Ensure continuity of care

• Conduct and document follow up calls with high risk clients as recommended (i.e., administer mental health screening tools).

• Manage appointments/schedule and medications (as well as refills)

• Prepare office for telehealth visit with doctor

• Prepare required paperwork prior to doctor appointments

• Pre-visit planning including but not limited to obtaining signatures on releases of information, request records from prior consults and therapies, vitals, current list of meds, drug allergies and history of tried/failed medications

• Abide by and maintain confidentiality rules and regulations

• Respond to quickly to referrals and requests for services

• Provides input and guidance to management on necessary policy development

• Maintain medical supplies inventory

• May act as point of contact or on-site supervisor when necessary

• Develops and leads staff education in psychiatric issues if needed

• Other duties as assigned

EDUCATION/WORK EXPERIENCE/QUALIFICATIONS:

• Completion of an associate, baccalaureate or master's degree program in Nursing. BSN strongly preferred

• Must have a current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Minnesota

• CPR for the professional rescuer or BLS for healthcare providers

• Knowledge of current nursing practices and techniques

• Ability to communicate clearly, both orally and in writing

• Ability to effectively participate on an interdisciplinary treatment team

• Ability to develop and maintain effective working relationships with clients, family members and co-workers

• Preferred two years of experience working in a behavioral health setting

• Preferred experience working with Native American clients and communities
