Starting: December 4, 2023

Closing: December 22, 2023

Primary Function:

Responsible for counseling, treatment plan development, care coordination, linkage to services, treatment plan reviews and progress updates. Develop an ongoing relationship for clinical support with the client and their supporters; life skills coaching, advocacy, treatment follow-up, disease management, and education are critical components to comprehensive substance abuse treatment. Reports to Clinic Supervisor. Salary: Starting DOQ, completion of 90-day Probation Period for full time position only.

ESSENTIAL RESPONSIBILITIES AND DUTIES:

• Provides individual substance use disorder counseling, Care Coordination, comprehensive assessments and referrals to assure clients have the tools necessary to achieve their treatment goals.

• Consults with supervisor to ensure the best possible services are in place for the client

• Assists clients through the clinic admission process. Includes arranging transportation, completing intake paperwork and offering support during the induction process

• Coordinates activities to bring services, groups, agencies, resources, or people together within a planned approach for providing comprehensive treatment

• Communicates progress to the client care team and updates treatment plan to assure quality care delivery for the client

• Assures that clients health record is complete, current, and compliant; including consent forms, releases of information, chemical use assessments, treatment plan, and clinical charting

• Documents communication with other providers regarding the client's progress

• Works collaboratively with medical staff to ensure patients are achieving treatment expectations

• Reports medical concerns immediately to nursing personnel to maintain client safety

• Maintains client confidentiality according to federal rules, regulations and tribal guidelines

• Participates in ongoing training and professional development

• Other duties as assigned

QUALIFICATIONS:

• GED/High School Diploma

• One (1) year related experience and/or training

• Licensed Alcohol and Drug Counselor (LADC) or UMICAD I, II, III certificate

• Possess a valid Minnesota driver's license and automobile insurance

• Proficient in Microsoft Windows and Office programs

• Must be a team player, dependable, and able to work well in a fast paced and demanding clinic environment

• Employment contingent upon successfully passing Pre-Employment Drug/Alcohol Test
