MINO-BIMAADIZIWIN WELLNESS CLINIC MINNEAPOLIS

PRIMARY FUNCTION:

The Mental Health Case Manager will provide culturally responsive Targeted Case Management services to adults living with severe and persistent mental illness (SPMI). The targeted case manager position supports clients in meeting their identified goals; this often includes achieving stability in meeting basic needs, connecting to needed financial and health supports, and engaging in social and community resources. The case manager role is to conduct holistic assessment to develop Individualized Community Service Plans in collaboration with each client to improve or maintain functioning across multiple domains. The Case Manager establishes and maintains connections to essential services, and advocates for and mobilizes resources for their clients. Reports to Director of Mino-Bimaadiziwin Wellness Clinic. ESSENTIAL RESPONSIBILITIES & DUTIES:

• Provide culturally responsive targeted case management (TCM) services to a designated caseload of clients

• Assess clients' biopsychosocial, vocational, and economic strengths, risks, and needs

• Visit clients in their home as well as in the community on a monthly basis to check progress, provide support and monitor mental health functioning

• In collaboration with clients, develop and monitor comprehensive service plans with measurable long- and short-term goals

• Facilitate access to needed medical, social, educational, vocational, financial and other necessary services. Advocate on behalf of clients for services not readily available

• Initiate appropriate referrals for needs and interests across functional domains

• Monitor attainment of client-identified goals. Coordinate services on an ongoing basis to meet the objectives identified in the service plan

• Maintain accurate and timely clinical records, correspondence, and reports as required

• Ensure timely completion of required court documentation for civil commitment proceedings

• Participate in community outreach, education programs, and trainings

• Attend supervision, agency staff meetings and in-services

• Additional duties consistent with program needs and professional development goals

EDUCATION/WORK EXPERIENCE/QUALIFICATIONS:

• Bachelor's degree in social work or human services related field or Case managers without a bachelor's degree must meet one of the requirements in clauses: have three or four years of experience as a case manager associate or be a registered nurse without a bachelor's degree, or be a person who qualified as a case manager under the 1998 Department of Human Service waiver provision and meet the continuing education and mentoring requirements or 2,000 hours of supervised experience in the delivery of services to adults with mental illness or A case manager without 2,000 hours of supervised experience in the delivery of services to adults with mental illness will or receive clinical supervision regarding individual service delivery from a mental health professional at least one hour per week until the requirement of 2,000 hours of experience is met; and complete 40 hours of training approved by the commissioner in case management skills and the characteristics and needs of adults with serious and persistent mental illness. Training will be completed upon hire. Ideal candidate will have experience working directly with Native American clients.

• Strong skills and knowledge of identifying issues and needs, and connecting with resources related to adults with severe and persistent mental illness

• Excellent assessment skills

• Proven oral and written skills to communicate effectively with a wide range of people, including de-escalation and crisis management

• Strong time management and task prioritization skills

• Experience providing services virtually or remotely preferred

• Proven ability to engage compassionately with clients

• Utilize harm-reduction and person-centered approaches with clients

• Ability to work creatively and collaboratively with colleagues internally and externally

• Proficient user of electronic health record systems for documentation and communication

• Proficient user of Microsoft Office programs, including Word and Excel

• Current Minnesota driver's license, clean driving record, auto insurance, and regular and consistent access to a motor vehicle
