SUPPORTIVE HOUSING ADVOCATE:
Full Time: 8:00 AM – 4:30 PM, Monday – Friday, with occasional evening/weekend hours
Wage: DOQ

Reports to: Supportive Housing Manager, Carol Priest
DUTIES AND RESPONSIBILITIES:
• Complete required training to become a Housing Stabilization Services (HSS) Provider and gain an understanding of the program.
• Assist potentially eligible participants to enroll in Medical Assistance and apply for HSS, including connecting with other providers to complete the required assessments.
• Support participants who are homeless in finding and securing housing according to their Housing-focused Person-centered Plan.
• Support participants who are housed to maintain housing according to their plan.
• Most HSS participants will be those residing in Red Lake Supportive Housing units. Additional participants may be engaged as time allows.
• Connect participants to health and human services; coordinate services with other providers to ensure access and eligibility.
• Address service and other needs of all household members.
• Conduct ongoing evaluation with participants of their progress and the effectiveness of services.
• Understand and maintain participant confidentially and data privacy standards.
• Create participant files including all relevant assessment, plan and evaluation information and update regularly as required for HSS.
• Understand and follow all requirement of the HSS program.
• Conduct outreach to inform community members of housing and service availability.
• Understand status as a mandated reporter of child or vulnerable adult maltreatment.
• Keep updated resource information on local and regional human services.
• Establish and maintain positive networking relationships with other service providers.
QUALIFICATIONS:
• Two years' experience working with people with mental illness and substance use disorders, and knowledge of services to assist them
• Two years' experience working with people who are homeless.
• Must be comfortable meeting with participants in their own homes, and using Covid-19 and other safety guidelines.
• Effective verbal and written communication skills, basic computer skills.
• Must pass a MN Department of Human Services background check and a pre-employment drug test.
• Must have reliable, insured vehicle and a valid Minnesota driver's license.
• High school diploma or GED.
