4-CASE INVESTIGATOR/CONTACT TRACERS

RED LAKE COMPREHENSIVE HEALTH SERVICES

OPEN: NOVEMBER 15, 2021-UNTIL FILLED

Primary Function:

A professional who performs the initial interview to recognize and track symptoms, provide educational resources and guidance, and obtain relevant contacts. Reports to Red Lake Chief Operating Officer, Clinical Director, salary; DOQ.

Tasks and Responsibilities will include the following but not limited to:

a. Preparing case interviews for infected patients from reporting sources, state and local health departments, or similar Tribal programs (such as Tribal Epidemiology Center (TEC) or Tribal Health Department) or urban Indian organizations (UIO);

b. Conducting patient interviews to identify symptoms to determine infection rate and potential risks.

c. Obtaining all frequent or recent contacts and location visited;

d. Maintaining appropriate discretion with the information;

e. Providing patient educational materials when necessary;

f. Documenting case findings and relaying information to identified local contact or authority; and/or

g. Initial call/case investigation/contact tracing, provide some oversight, coordinating effort of the Contract Tracers/Public Health Specialist.

Minimum Qualifications/Experience Requirements:

• Health care, public health or related experience; and /or

• Case Management experience;

Preferred Qualifications/Experience Requirements:

• Qualified candidate who is local to the community or reservation;

• Candidate who can provide onsite support.
