ATTENTION:

In anticipation of an upcoming 2020 Trust Fund Distribution. The Enrollment Department has set the following deadlines:

Deadline to submit Applications for Enrollment: November 12, 2020 at 5:00 p.m.

a) A Completed Notarized application

b) Certified State issued Birth Certificate

c) Legal documentation of name change

d) All other information as stated on the enrollment application

*Please note: enrollment applications received after the deadline will be reviewed at the next quarterly enrollment meeting in March 2021. Incomplete applications will not be accepted or processed

Deadline to update Proof of Guardianship forms: November 23, 2020 at 5 p.m.

(must be one of the following)

a) Current court order

b) Custody order

c) Relative care verification

d) MFIP verification

Deadline to update your address: November 24, 2020 at 5:00 p.m.

Please note: (if you have moved within the last year, you must update your address by this date.)
