OUTREACH WORKER (A MOTHER'S SACRED GIFT)

CHEMICAL HEALTH PROGRAMS

PRIMARY FUNCTION

Responsible for assisting to plan and carry out personal change in the use of alcohol and other drugs for clients to deal with related life issues including age appropriate personal growth and development. Reports to Project or Executive Director, Salary: DOQ

ESSENTIAL RESPONSIBILITIES AND DUTIES

• Communicate and maintain good rapport with outside resources while providing care

• Communicate with other professionals in regard to client treatment and services to assure comprehensive quality care for the client

• Assure that all records are complete, current, and available in the client's file; maintain records of communication with other agencies about the client

• Collect and maintain data for reports to funding agency

• Provide information and education about chemical health to clients as individual needs require

• Intervene in client crisis and use circumstances to benefit the client's treatment

• Refer to other resources when that is in the best interest of the client

• Use consultation and case management to improve client treatment and services

• Follow up to determine how client is doing in recovery

• Must be punctual in work attendance

• Maintain strict confidentiality at all times

• Performs other duties as assigned

EDUCATION/WORK EXPERIENCE/QUALIFICATIONS

• High School Diploma or G.E.D Certificate

• One (1) year experience and or training or equivalent experience

• Must have at least one (1) year of being chemically free immediately before hiring

• Have a valid driver's license and auto insurance

• Knowledge of Microsoft Office programs

• Must possess excellent customer service skills and be dependable

• Must refrain from all mood altering chemicals

• Must be able to follow orders precisely and cooperate with other staff

• Must understand the Red Lake Tribal Personnel Policies & procedures and the Chemical Health Programs Chain of Command

• Employment contingent upon successfully passing Pre-Employment Drug/Alcohol Test and Background Check
