CHILDREN'S MENTAL HEALTH CASE MANAGER

FAMILY & CHILDREN SERVICES

PRIMARY FUNCTION

To provide case management duties for children diagnosed as Severely Emotionally Disturbed (SED). To provide access to care and coordinate services for the families given the child's needs. To work collaboratively with other person (s) and agencies to ensure the child's needs are met. Attend meetings and conduct functional assessments. Reports to Executive Director, full-time position with benefits, salary; DOQ.

ESSENTIAL RESPONSIBILITIES AND DUTIES

• To meet and collaborate with individuals and families

• To assess and identify individual and family needs

• To keep up-to-date on all required paperwork

• To be flexible in working with individuals and families

• To identify and assist families in accessing various services

• Able to maintain STRICT confidentiality

• Assume other duties and responsibilities as assigned

EDUCATION/WORK EXPERIENCE/QUALIFICATIONS

• Must hold a bachelor's degree in one of the social sciences; social work, sociology, criminal justice, or psychology

• Knowledge of normal and abnormal child development

• Be aware of community based services

• Computer competency and good record keeping skills

• Must be willing and able to meet additional training hours (if needed) as well as ongoing training requirements each year.

• Must be a team player and able to work collaboratively with other person's, agencies, and service providers

• Must be able to manage crisis situations and flexible working conditions

• Must have a valid Minnesota Driver's license and auto insurance

• Employment contingent upon successfully passing Pre-Employment Drug/Alcohol Test and Background Check
