Veterans Outreach Case Manager - Red Lake Homeless Shelter

Full Time: 8 AM – 4:30 PM, Monday – Friday occasional evening/weekend hours

Wage: $12.00 per hour

Reports To: Red Lake Homeless Shelter Executive Director and MACV-Duluth Programs & Outreach Manager

Case Manager Duties include:

• Position will be responsible for interviewing veterans (client screening/intakes), providing information and referral over the phone and in person for new clients and those contacting the office for assistance.

• Assists with case management activities of the region by assisting with tasks related to appointments, referrals, follow-up and other services.

• Assists the Minnesota Assistance Council for Veterans Program & Outreach Manager in tracking program expenses to ensure they do not exceed the program’s budget as well as assists with quarterly reporting.

• Gathers and enters program data for evaluation as requested, including HMIS

• Assessing client strengths and identifying barriers

• Developing specific, individualized service plans based on the assessment

• Connecting participants to necessary services by coordinating services and working with other service providers to ensure access, eligibility and follow-through by participant and provider

• Conduct ongoing evaluations of the participant’s progress through the plan, reassessment and adjustments to plan

• Maintaining complete and detailed documentation as required

• Works with other program staff to ensure compliance with grant outcomes and performance expectations to ensure established goals are met.

• This position will also assist the Program & Outreach Manager in direct outreach activities to include assistance with regional fundraising projects as needed.

• Per MOU with Minnesota Assistance Council for Veterans and Red Lake Homeless shelter – Goal of SSVF grant is 40 homeless veterans, to be entered and tracked in HMIS data system. HMIS training to be a part of this position.

Minimum Qualifications Desired:

• High School Diploma/GED

• Valid Driver’s License

• Military service a plus but not required

• Strong verbal and written communication skills along with strict attention to detail.

• Must be able to multiplex in a fast moving environment and be adaptable to change.

• Self-motivated needing minimal supervision. Position will be located in satellite office location.

• Experience working with individuals who are homeless, chemically dependent or those having a mental illness.

• Understanding of addiction/recovery and 12-Step programs a plus.

• Veteran, or knowledge of veteran issues, preferred.

• Proficiency in Microsoft Office, specifically Microsoft Outlook and ability to effectively organize folders/files. Database experience preferred.
