JOB DESCRIPTION: Dementia Outreach Advocate .5 FTE, Grant funded position, minimum 15-month commitment

ACCOUNTABLE TO: Executive Director

GOALS: This employee will provide community outreach to people living with dementia and their caregivers, with a special focus on Native American people and tribal communities. They will provide caregiver coaching services according to MN Board on Aging and REACH intervention practices. They will offer culturally sensitive memory screenings to people in the community.

QUALIFICATIONS:

Experience: Bachelor's Degree or equivalent experience in a field related to target population: human services, nursing, etc. Extensive work and/or life experience with Native American people. Knowledge of resources in local tribal communities.

Skills: Computer skills including Word, Excel, Internet, Email. Excellent communication skills with diverse populations, including Native American and LGBTQ elders.

Ability to effectively lead a community action team and make public presentations.

Personal: Sincere desire to serve, openness and flexibility, enthusiasm for working collaboratively on community issues. Willingness to implement the organization's mission and philosophy. Must pass background checks and have current driver's license, insurance and reliable transportation required. Must have reliable phone for out of office contact.

TIME COMMITMENT: An average of 20 hours per week which will include some evening and weekend hours. Some overnight travel will be required.

SALARY & BENEFITS: This is an hourly wage position at $15.00/hour. Travel expenses and mileage will be reimbursed. Paid vacation, sick leave and holidays pro-rated to .5 FTE.

This is a grant funded position and may end after 15 months.

DUTIES

The Dementia Outreach Advocate will seek to implement delivery of services through:

Community Outreach:

1. Completing Dementia Capability Training online and in person on February 22, 2016 in St. Paul.

2. Coordinating the work of the ACT on Alzheimer's Native American subcommittee to achieve the goals of the project and MBA Dementia Grant.

3. Planning and implementing a community outreach campaign targeting people with dementia and their caregivers, with special emphasis on Native American and low income people.

4. Plan and carry out Alzheimer's Association Volunteer Community Educator training. Participate in training.

5. Work with Alzheimer's Association and Native dementia experts to develop and share culturally specific dementia materials.

6. Work with Action Team to plan and carry out memory screening events in diverse communities using culturally sensitive tools. Connect identified individuals with medical practitioners and follow up on results.

7. Help identify and compile a list of medical practitioners in diverse settings/locations who will accept new patients for complete evaluations including cognitive measures.

CAREGIVER COACHING:

1. Attend Caregiver Coaching training in March 2016.

2. Work with the ACT Native American subcommittee to plan, recruit for, and participate in the REACH into Indian Country training, hopefully with Leech Lake, Red Lake, and White Earth Nations and the MN Indian Area Agency on Aging.

3. Provide caregiver coaching and REACH services to caregivers, with a special emphasis on Native American caregivers.

4. Coordinate a network of all area providers trained in REACH for regular consultation meetings and email communications.

5. Help plan and implement a monthly Memory Café for people with dementia and their caregivers.

6. Facilitate Caregiver Support Groups as requested.

GENERAL:

1. Attend all other training as required.

2. Record and report activities and data as needed for grants.

3. Other duties as assigned.
