Primary Function:

The Healing Wellness Court (HWC) Tribal Youth Case Manager will be responsible to work with Red Lake youth who have committed a juvenile offense and/or have drug or substance abuse problem, and have been referred to the Red Lake HWC. Reports to the HWC Coordinator/Abinoojiiyag Noojimoo Wigamig Director, full-time position w/benefits, salary; $12.00 hourly.

Essential Responsibilities & Duties:

• Attends and participates in all scheduled HWC youth and family meetings.

• Is responsible for maintaining and updating youth client files for the official record, ensuring that all information is complete and accurate. This also includes mailing and obtaining signed release and information forms to/from youth parents

• Communicates with the Healing to Wellness Court regarding youth behavior/progress, special needs, compliance with orders of the HWC and expectations of youth while participating in the Healing to Wellness Court. As needed, or directed by HWC Coordinator/Abinoojiiyag Noojimoo Wigamig Director, transports youth to HWC hearings and other appointments

• Responsible for developing a holistic case management plan based on the agreements between the HWC and the youth and family

• Responsible for documenting and monitoring the HWC case plan for the youth and family

• Responsible to providing, linking and monitoring services with treatment providers, school counselors, juvenile probation staff, and other health/wellness service provider

• Prepares youth Progress Reviews, and other reports as ordered by the HWC, as well as corresponds with the HWC and youth parents regarding such reports

• The HWC Tribal Youth Case Manager will be responsible to plan and coordinate bi-weekly follow up sessions with the youth and the family

• The HWC Tribal Youth Case Manager will also provide compliance monitoring, and administers drug and breathalyzer testing as required

• Other duties as assigned

Qualifications:

• Must be 21 years of age

• A.A or B.A. Degree; or a combination of education, work experience and training

• Knowledgeable of Word documents and Excel spreadsheets

• Possess excellent working knowledge of programs such as Chemical Health, Family & Children Services and Mental Health, Corrections and Rehabilitation Program operation(s) for Youth in juvenile justice settings

• Possess the ability to establish and maintain effective working relationships with people of various economic, educational and cultural backgrounds

• Must have knowledge of Ojibwe history, culture and value system.

• Possess excellent written and oral communication skills

• Possess valid Minnesota Driver's License and current active Minnesota automobile insurance

• Must pass state and federal background investigation

• Employment contingent upon successfully passing Pre-Employment Drug/Alcohol Test
