Bemidji Community Health Center Community Forum - Friday, December 2, 2011

In September 2011, our community was awarded a Health Center Planning Grant through HRSA and the Affordable Care Act funds to plan for a community health center that will provide comprehensive, high-quality preventive and primary health care to patients regardless of their ability to pay. At the end of the planning phase, the plan and design will be submitted for consideration as a Federally Qualified Health Center (FQHC), FQHC look-alike or a similar integrated care network. The 10-month planning process will take a collaborative approach encompassing the commitment of service providers, health care professionals, clients, Minnesota Association of Community Health Centers, nonprofit organizations, and many others who share a vision to bring three aspects of health care - primary, mental and dental care - together to assure that the uninsured, underinsured and most vulnerable people in our region are served with a medical home and care that is:

 culturally competent;

 patient centered/provides care coordination;

 eliminates/addresses disparities and barriers;

 uses a team-based approach in delivering services; and,

 utilizes Ruby Payne's Bridges Into Health strategies to reduce inequities and improve health, and, provides a social and physical environment that promotes good health for all.

The planning process for a Community Health Center is underscored by a strong commitment to ensure the process remains open to the entire community. We welcome those who share the values and priorities of the community health center, want to be involved, have concerns, or are simply interested in learning more.
