Opportunity to participate in historic research, in Bemidji this Saturday

This year Beltrami County and Northern Minnesota have the opportunity to make history by hosting an enrollment site for the Cancer Prevention Study-3 (CPS-3). The CPS-3 is a study that is fully funded by the American Cancer

Study and the ultimate goal of the study is to find how factors such as our genetics, lifestyles, and environment can cause and prevent cancer and to ultimately help to eliminate cancer as a health risk for this and future

generations.

To be eligible to enroll in the study individuals must met the following criteria:

*be between the ages of 30-65

*have never been diagnosed with cancer

*willing to make a long term commitment to the study; 20-30 years filling out questionnaires periodically over that time

The enrollment process is broken down into 2 phases. The first phase takes place at the Beltrami Relay for Life on Saturday July 23, 2011 between the hours of 4-8pm. During this phase eligible participates will fill out a short questionnaire, provide a confidential waist measurement, and provide a ONE TIME blood draw taken by trained

phlebotomist. This phase is estimated to take 30 minutes.

The second phase, and the most critical part of enrollment is the completion of a more detailed questionnaire that will be mailed to you at your home the week. Once you have completed this questionnaire (which can also be

completed online) and it is returned to the American Cancer Society you will be fully enrolled in the study.

Periodically over the course of the next 20-30 years you will be mailed follow up questionnaires tracking your health and medical outcomes. These questionnaires are estimated to take 45-60 minutes to complete. Over the

course of the 20 year study you will be spending only 8-10 hours of your time completing the questionnaires.

Beltrami County is only 1 of 4 counties in Minnesota that has been chosen to make history! This is the ultimate way for you to FIGHT BACK against a disease that takes too much. In the future when you hear that the CPS-3 has found a way to prevent a type of cancer you can be proud to know that you were part of that finding.

Please consider enrolling in the Cancer Prevention Study-3 on July 23, 2011 at the Sanford Event Center between 4-8pm. You do not have to be a participant of the Relay for Life to enroll in the study.
