COORDINATOR (BE WELL BABY & MOM)

CHEMICAL HEALTH PROGRAMS

PRIMARY FUNCTION:

Responsible for the implementation/further development of; an existing care plan, service referral, linkage, monitoring of client progress. Including an ongoing needs assessment and providing life skills advocacy, treatment follow-up, disease management, and education. Reports to Executive Director, full-time w/benefits, salary; DOQ.

ESSENTIAL RESPONSIBILITIES & DUTIES:

• Provide Coordination, consultation, and referral to assure that client has access to all resources needed to achieve goals under the treatment plan

• Coordinates the activities to bring services, agencies, resources, or people together within a planned framework of action toward the achievement of established goals

• Communicates behaviors to placing agency and recommend case plan changes

• Coordinates with supervisor for the best services for the client

• Coordinates services between the Red Lake Pregnant Women and Families Program and the applicable community on behalf of the client and their families

• Ability to develop an ongoing relationship of clinical support with patient/client and his/her supporters

• Ensure compliance with all grant goals and objectives

• Submit all documentation required by funding agency

• Understand and abide by confidentiality rules and regulations

• Complete training on federal alcohol and drug confidentiality laws within 60 days of hiring

• Participate in ongoing training and professional development

• Other duties as assigned

QUALIFICATIONS:

• GED or High School Diploma

• Two (2) years in related experience and/or training or equivalent combination of education and experience

• At least one (1) year continuously chemically free immediately before hiring

• Possess a valid driver's license and auto insurance

• Knowledge of Microsoft Office programs

• Must possess excellent customer service skills and be dependable

• Must refrain from all mood altering chemicals

• Must be able to follow orders precisely and cooperate with other staff

• Must understand the Red Lake Tribal Personnel Policies & procedures and the Chemical Health Programs Chain of Command

• Employment contingent upon successfully passing Pre-Employment Drug/Alcohol Test and Background Check
