PRIMARY FUNCTION:

The Community Health Nurse is responsible for providing diagnostic, preventive and rehabilitation nursing care services to individuals and families in promoting physical and psychological health and well-being. This includes overall health care needs of patients in collaboration with other community service providers (i.e. health education, nutrition counseling, social services, behavior health services, environmental health, chemical dependency services, vocational rehabilitation, etc.) Reports to the Director of Community Health Nursing Services (CHNS). Full-time with benefits, salary, DOQ.

ESSENTIAL RESPONSIBILITIES AND DUTIES:

• Participate in planning, development and implementation of additions to the program.

• Responsible for providing skilled nursing care and case management services to patients based on a plan of care developed in collaboration with the PCP, patient and family which identify health needs obtained through interview, record analysis and medical examination of the patient.

• Ability to adequately assess and reassess pain. Utilize appropriate pain management techniques. Educate the patient and family regarding pain management.

• Provides information to clients and families about the extent and limitations of services available through the CHNS program.

• Responsible for planning and coordinating nursing care services for patients and families with other health professionals.

• Adapts hospital procedures to home situations to provide skilled care to patients regarding intermittent professional services.

• Responsible for planning with the family and health provider care which is feasible within the physical and emotional resources of the family.

• Responsible for providing nursing supervision for CHTs involved in the care of the patient.

• Ensures that PCP orders are communicated to other health care team members in a timely, accurate and thorough manner.

• Responsible for observing and assessing the potential dangers of disabling conditions and indicates preventive and corrective measures to the patient.

• Responsible for observing and evaluating patient health status and reports to the patient’s PCP, the patients’ physical and emotional condition, reactions to drugs or treatment, refusal for care and interprets social and physical factors in the home that affect patient care. Function as a team member with colleagues.

• Maintains current knowledge of community resources. Identifies and assesses appropriate external and internal resources to meet patient and/or family needs in order to facilitate optimal patient outcomes while home care services are being done.

• Supports and maintains a culture of safety and quality service.

• Coordinates follow-up and appropriate treatment of patient for diagnosed communicable diseases or exposure to them and applies measures for prevention and control as required by Tribal Law and the Minnesota Department of Health.

• Performs diagnostic tests as authorized and interprets findings of test to patient and family. Obtains laboratory specimens when indicated. Gives preventive immunizations or treatment under medical supervision.

• Responsible for observing the environment in the home and community to identify elements conducive to health hazards and contacts community resources and programs to help eliminate or modify the potential hazards.

• Responsible for coordinating and/or providing health screening programs for the community (i.e. immunizations, blood sugars, etc.)

• Documentation is performed in the patient’s EHR. Additional signatures are added to notes for other providers to increase communication.

• Prepares a monthly report to be submitted to the CHNS Director as scheduled in a timely manner.

• Participates in Red Lake community workshops, staff in-services, annual health fairs and other activities.

• Assists the CHNS Director in the development of educational materials for patients and the community.

• Participated on Service Unit committees as directed by the CHNS Director.

• Other duties as assigned.

PROFESSIONAL SKILLS AND KNOWLEDGE REQUIRED:

• Adheres to dress code and must be neat and clean.

• Completes education requirements to maintain current licensure requirements.

• Maintains and ensures patient/family confidentiality at all times.

• Wears identification badge while on duty. Works at maintaining a good rapport and a cooperative working relationship with the PCPs, Service Unit Staff, Extended Care Facility (ECF) staff, CHNS staff and Tribal Programs.

• Represents the CHS CHNS programs in a positive and professional manner on and off duty.

• Complies with all organizational policies.

• Communicates the mission and goals of the organization.

• Knowledge of the mission statement, program requirements and policies of the health care delivery system of Comprehensive Health Services.

• Knowledge of professional community health nursing principles, practices and procedures in providing nursing services in the clinic, home, school and community environment.

• Knowledge of drugs and biologicals and the desired effect in the treatment of diseases as well as undesired side effects.

• The ability to provide counseling in preventative care to healthy individuals through increased physical activity, nutrition and a healthy lifestyle.

• Skill in making physical and psycho social assessments of patients and families recognizing the range of “normal” and the manifestation of common abnormalities.

• Knowledge of quality assurance and the evaluation process.

• Assists the Minwanjige WIC program Coordinator to certify nutritional services based on state and federal requirements.

• Assists staff from other agencies (state, local and tribal) by coordinating nutritional services related to these programs.

• Identifies nutritional problems, establishes short and long range objectives, sets priorities, plans methods to be used and evaluates progress.

• Makes home visits for as referred from various tribal programs or health providers.

• Submits a monthly report and other reports as assigned.

• Attends meetings as required by the State Early Intervention Committee (EIC) program and other meetings as assigned

• Performs all other duties as requested by the Director of Community Health Nursing Services or Tribal Health Director in order to accomplish the goals/objectives of the Red Lake CHS.

EDUCATION/WORK EXPERIENCE/LICENSURE REQUIRED:

• Graduate of an accredited school with 4 year BSN program

• Two years nursing experience as a nurse working in the community health. Preferred but not required.

• PHN certificate

Maternal Child Health (MCH) Community Health Nurse (CHN)

PRIMARY FUNCTION:

The MCH CHN is responsible for providing diagnostic, preventive and rehabilitative nursing care services to infants/children up to three and their families in promoting physical and psychological health and well-being. This includes the overall health care needs of patients in collaboration with other community service providers, i.e. home care, health education, nutrition counseling, social services, behavioral health services, environmental health, chemical dependency services, vocational rehabilitation, etc. Reports to the Director of Community Health Nursing Services (CHNS). Full-time with benefits, salary, DOQ.

ESSENTIAL RESPONSIBILITIES AND DUTIES:

• Responsible for teaching pre-natal curriculum that is available for all pregnant women.

• Recruits pregnant women during Pre-natal Clinic for Childbirth classes, Family Spirit program and CHNS newborn visits.

• Coordinates referrals for missed pEr-natal appointments or other high risk conditions.

• Provides all referred newborn infants and their mothers with an assessment home visit within the first two weeks.

• Responsible for providing skilled nursing care to children ages 0 to 3 years based on a plan developed in collaboration with the PCP and parents that identifies health needs obtained through interview, records analysis and medical examination of the patient.

• Provides information to families about the extent and limitations of services available through the CHNS program.

• Responsible for planning and coordinating nursing care services for children and families with other health professionals.

• Responsible for providing case management services to children in collaboration with community services providers (i.e. transportation, Social Services, Behavioral Health, Chemical Dependency, etc.).

• Responsible for providing nursing supervision for CHTs involved in the care of the infant/child.

• Responsible for observing and assessing the potential dangers of disabling conditions and indicates preventative and corrective measures for child and family.

• Responsible for observing and evaluating child’s health status, reports to child’s health provider their physical and emotional condition, reaction to drugs or treatments and interprets social and physical factors in the environment that affect child care.

• Responsible for planning with the family and health provider care which is feasible within the physical and emotional resources of the family.

• Reports suspected child abuse and neglect to Family and Children Services per protocol.

• Attend the Child Protection Team meeting held weekly at Family and Children Services

• Responsible for teaching basic principles of child development, parenting principles and anticipatory guidance to families with children.

• Provides health education for families utilizing the Family Spirit Program.

• Assists families in accepting responsibility for their own care. Guides families toward self-help in recognition and solution of physical, emotional and environmental health problems.

• Performs diagnostic tests as authorized and interprets findings of these tests to children and families.

• Responsible for obtaining laboratory specimens when indicated and follows program policy for blood borne pathogen control.

• Coordinates follow-up and appropriate treatment of patient for diagnosed communicable diseases or exposure to them and applies measures for prevention and control as required by Tribal Law and the Minnesota Department of Health.

• Responsible for observing the environment in the home and community to identify elements conducive to health hazards and contacts community resources and programs to help eliminate or modify the potential hazards.

• Responsible for documenting in the patient’s EHR for children seen intermittently.

• Prepares a monthly report to be submitted to the CHNS Director as scheduled in a timely manner.

• Participates in Red Lake community workshops, staff in-services, annual health fairs and other activities.

• Assists the CHNS Director in the development of educational materials for the community and families.

• Participates on Service Unit committees as directed by the Community Health Nursing Director. Perform all other duties as requested by the CHNS director or Tribal Health Director to accomplish the goals/objectives of the Red Lake CHS.

• Other duties as assigned.

• Knowledge of drugs and biologicals and the desired effect in treatment of disease as well as undesired side effect.

• Knowledge of an ability to provide counseling in preventive care for infants and families by guidance in nutrition, common illnesses, anticipatory guidance, child growth and development, parenting skills, behavioral health, etc.

• Skill in making physical and psychosocial assessments of individuals and families. Recognizing the range of “normal” and the manifestation of common abnormalities.

• Knowledge of quality assurance and the evaluation process.

EDUCATION/WORK EXPERIENCE/LICENSE REQUIRED:

• A two year RN, AS or ADN degree; or a four year BSN or PHN degree in nursing.

• A minimum of two years experience in Community Health Nursing.
