REGISTERED NURSE – MDS COORDINATOR
JOURDAIN/PERPICH EXTENDED CARE CENTER
OPEN: January 30, 2015
Closing: February 20, 2015 @ 3:00 p.m.
Purpose of Position:
Responsible for assuring the timely completion of accurate assessments and interdisciplinary care plans that meet Federal/State Guidelines. Included is identifying resident acuity and needs, helping to determine specific care and communicating needs/expectations to families and responsible parties. Full-time position with benefits, reports to the Director of Nursing, salary DOQ.
Duties and Responsibilities:
• Meet with Nursing staff as well as support personnel in planning the department's services, programs, and activities.
• Develop and maintain a good working rapport with inter-department personnel, as well as other departments within the center to assure that Nursing Services and activities can be adequately maintained to meet the needs of the residents.
• Assist in developing, implementing and coordinating department policies and procedures, MDS's, resident/nursing care plans, nursing procedures manuals, develop and maintain nursing objectives and standards.
• Assures that established infection control procedures and isolation techniques are followed at all times.
• Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
• Review the nursing requirements of each resident admitted and assist the attending Physician in planning the residents' Plan of Care.
• Assure that a stock level of medications, medical supplies, equipment, etc., is maintained at all times.
• Review nursing notes to assure that they are informative and descriptive or the nursing care provided of the resident's response to the care.
• Make rounds with attending Physicians and Director of Nursing as requested or as may be necessary.
• Make routine rounds to assure that nursing service personnel are performing required duties and to ensure that appropriate nursing care is being rendered to meet the needs of residents.
• Develop treatment plans for patients that comply with MDS guidelines.
• Responsible for completing the MDS paperwork, submitting it on time and making sure it is compliant with current regulations, which have the potential to change each year.
• Maintain the strictest confidentiality at all times.
• Other duties as assigned.
Qualifications:
• Current license to practice as a Registered Nurse in Minnesota.
• Two years of specialized supervisory/managerial capacity experience in an extended care center.
• Employment contingent upon successfully passing Pre-Employment Drug/Alcohol Test and Background Check.
