DEADLINE TO SUBMIT ENROLLMENT APPLICATIONS

FRIDAY - NOVEMBER 8, 2013 AT 5:00 PM

Only complete applications will be accepted

A CERTIFIED STATE ISSUED BIRTH CERTIFICATE IS REQUIRED

Recognition of Parentage can be completed at the Enrollment Dept.

Red Lake Parents must be on the certified birth certificate.

If one parent is from another reservation, the Red Lake Enrollment Dept. cannot guarantee that other enrollment offices will respond in a timely manner.

NO COPIES OR FAXES WILL BE ACCEPTED
